Involving patients in service improvement

摘要: This article describes the experiences of two multidisciplinary teams that took part in a national initiative to improve care through implementation research evidence. Both worked with patients chronic illnesses. The required regular admissions hospital for treatment and care, had reported difficulties when moving between different services.
