Use of the nine-step inflation/deflation test as a predictor of middle ear barotrauma in sports scuba divers.

摘要: Middle ear (ME) barotrauma, the most common disorder encountered in diving, results from inadequate pressure equilibration between ME and ambient environment. Eustachian tube function plays a key role pathogenesis of barotrauma. This study was designed to investigate predictive value efficiency tympanometric tests (Valsalva test, Toynbee test nine-step inflation/deflation test) predicting barotrauma 44 ears 22 sports scuba divers who had normal audiometry, tympanometry otorhinolaryngological examination without previous history disease. The were counselled refer investigators if any symptoms occurred during and/or after diving. All symptomatic examined within 24 hours diving by one investigator unaware pre-symptomatic results. Decision matrix analysis applied for with Values also evaluated battery 'Lax' (positive on A, B or C) 'Strict' criteria. found be efficient (93%) highest values (PPV 83%; NPV 95%), whereas Valsalva unreliable 0% 25% respectively). Combining into two-test strict approach increased PPV (100%). It appears that is reliable method sufferers, especially when test. may have evaluation candidates routine otorhinolaryngological, audiological evaluation.
