摘要: La hernie diaphragmatique n’est plus une urgence chirurgicale. Elle est de reanimation qui va permettre mesurer l’importance l’hypertension arterielle pulmonaire et l’hypoplasie pulmonaire. Une fois que ces parametres auront ete evalues au mieux (surtout) l’enfant aura stabilise, il pourra etre opere. Les voies d’abord les techniques ont evolue cours des dernieres decennies, avec l’apport la thoracoscopie laparoscopie. fermeture larges defects passe par l’utilisation protheses ou d’un lambeau musculaire. Du fait reflux gastro-œsophagien souvent associe, dans formes un large orifice, geste antireflux peut realise le meme temps operatoire. surveillance a long terme justifiee risque brides, recidives croissance asymetrique cage thoracique pouvant conduire constitution veritables scolioses.
