摘要: Summary Reports of anaesthetic misadventures were regularly collected in the Anaesthetic Department a district general hospital, to identify recurring problems. Eighty-one misadventures, none which had serious outcome, reported during 6-month period, 8312 anaesthetics administered. Human error was more frequently responsible than equipment failure, and failure perform normal check factor most associated. Local hazard warnings circulated when necessary members Department, reports formed basis departmental discussion teaching.
