摘要: Summary Background Results from previous studies show that the cognitive ability of offspring might be irreversibly damaged as a result their mother's mild iodine deficiency during pregnancy. A reduced intelligence quotient (IQ) score has broad economic and societal cost implications because affects wellbeing, income, education outcomes. Although pregnancy lactation lead to increased needs, no UK recommendations for supplementation have been issued pregnant women. We aimed investigate cost-effectiveness versus women in mildly moderately iodine-deficient population which population-based programme—for example, universal salt iodisation—did not exist. Methods systematically searched MEDLINE, Embase, EconLit, NHS EED linked IQ income published all languages until Aug 21, 2014. took clinical data relating effect on children aged 8–9 years primary research. decision tree was developed compare treatment strategies tablet form with UK. Analyses were done health service perspective (analysis 1; taking direct costs into account) 2; value an point itself account), presented terms (in sterling, relevant 2013) per gained offspring. made data-supported assumptions complete these analyses, but used conservative approach limited benefits overestimated its potential harms. Findings Our systematic search identified 1361 articles, eight assessed calculate monetary point. discounted lifetime additional based earnings estimated £3297 (study estimates range £1319 £11 967) cohort. Iodine saving both (saving £199 woman [sensitivity analysis –£42 £229]) £4476 £540 £4495]), net gain 1·22 points each analysis. Base case results robust sensitivity analyses. Interpretation is potentially saving. This finding also 1·88 billion people 32 countries worldwide. Valuation should consider non-earnings benefits—eg, associated higher germane earnings.
