摘要: End-of-life care for patients with cardiovascular illness living in skilled nursing facilities (SNF) is associated challenges related to prognosis and symptom management. This chapter provides an overview of the components end-of-life disease. It includes advanced planning, shared decision making, management, bereavement appropriate this complex population. Among approximately two million SNF residents, disease largest diagnostic category heart failure common. Symptoms are burdensome negatively affect quality life. Symptom management maximized medical guideline-based therapy as tolerated important from time diagnosis end-of-life. As determining among difficult, using uncontrollable symptoms a trigger may be prudent. An interdisciplinary approach skills nursing, medicine, social work, chaplains volunteers recommended resident family satisfaction focuses on needs by encouraging supporting decision-making.
