摘要: The importance of a specific transition process is recognized by many health organizations. Got Transition, cooperative endeavor aimed at improving the from pediatric to adult care, developed Six Core Elements defining basic components care support. In this article, we review presenting model that combines resident quality improvement and training. Internal Medicine-Pediatrics residency program, ambulatory training for residents takes place in combined clinic. Aligned with Elements, program has crafted disseminated policy practice, designed portable summary template electronic medical record (EMR), created EMR tools assessing readiness setting goals, formed registry patients, audited charts.
