摘要: Purpose A pharmacist-managed chronic pain clinic (PMCPC) in a primary care setting is described. Summary As providers (PCPs) may be unprepared or lack time to manage high-risk patients receiving opioids for nonmalignant pain, alternative models of are needed. The University Colorado PMCPC integrated into an internal medicine outpatient clinic. staffed by 1 clinical pharmacist, with pharmacy students and residents also performing duties. team reviews health records determine eligibility services documents referral requests the electronic record (EHR); on PCP acceptance referral, assumes responsibility patient’s management under collaborative practice agreement. Using drug therapy (CDTM) protocol, conducts patient assessments, including assessment signs aberrant drug-taking behaviors; provides initial ongoing counseling education; makes recommendations opioid dosage adjustments regimen additions discontinuations. Experience at date indicates that model feasible accepted PCPs patients. Conclusion based was established improve who prescribed period 3 months longer. Clinic referred through EHR managed pharmacist CDTM protocol.
