Arthroscopic treatment of transchondral talar dome fractures

摘要: Abstract Flake fractures of the talar dome involving cartilaginous surface and subchondral bone have been called transchondral fractures, a term specific for both anatomical traumatic etiology this lesion, more descriptive than previously used “osteochondritis dissecans” talus. The diagnosis is made by standard ankle radiographs lesion should be suspected in all “sprained ankles” that continued disability following treatment. Surgical treatment has recommended Stage 3 4 (detached/displaced fragments). removal symptomatic lesions has, past, involved an arthrotomy exposure with fracture fragment curetting drilling donor bed. Posteromedial are often hard to reach through approach may require osteotomy medial malleolus. use arthroscope confirmation surgical proved beneficial these fractures. technique allows direct visualization pathology adequate debridement minimal iatrogenic trauma early mobilization return preinjury status. series includes 10 ankles nine patients eight posteromedial lesions, six two 4, anterolateral 4. All were treated arthroscopically loose curettage osteochondral Subjective objective follow-up at minimum 6 months revealed good results ten patients.
