摘要: Antiarrhythmic and hemodynamic effects of i.v. disopyramide phosphate (1.7 mg/kg b.wt. over 2 min) have been studied in nine patients, several various degrees cardiac decompensation, with sinus rhythm persistent ventricular ectopic beats (VEBs). In one case primary cardiomyopathy, greater than 30 VEBs/min, (DE) abolished the arrhythmia for min, but precipitated brief dysponea. Other side-effects were tolerable mainly attributable to anticholinergic drug. DE either or significantly reduced all cases. For only patient showed VEBs, three patients no VEBs seen hours. Changes output pulmonary artery (PAP) central aortic pressures measured eight patients. Negative inotropic indicated seven by an increased diastolic PAP/stroke volume ratio a decreased (dp/dt)max. Patients high control values PAP marked reductions output, stroke work. predicting myocardial depressant DE, seemed be superior venous pressure, index systolic time intervals. Mean arterial pressure 5 10 min after drug administration significant change, indicating that vasoconstrictor reflexes well preserved, level above value was reached from 20th min. It is concluded potent suppressing exerts negative may clinical importance. The optimal antiarrhythmic dose probably lower used present study.
