摘要: Spasmodic (spastic) dysphonia (SD) is considered by some to be a neurologic syndrome and others symptom complex of multiple etiologies, psychogenic. A case 26-year-old female psychiatric nurse with psychogenic SD (PSD) presented. The was alleviated within one session voice therapy. Psychogenic etiology established the author, based on three diagnostic criteria--symptom incongruity, reversibility, psychogenicity. Seven nationally recognized experts listened audio-recorded samples patient's pre- posttherapy during conversational speech. agreed that characterized it as staccato-like speech, effortful phonation, interrupted flow speech; six intermittent arrests (voice stoppages); five hoarse-harsh voice; four waxing waning, strained-strangled phonation. These are often described salient features SD. Nevertheless, disagreed among themselves whether characteristic should labeled such. author argues long characteristics pathophysiologic findings constitute not well delineated, diagnosis symptoms alone, patients or poorly understood disorders likely misdiagnosed organic (neurologic) thus subjected undue medical treatment. also debate over can resolved if syndrome, PSD nonorganic phonatory disorder mimics defined on.
