摘要: Various definitions of health and mental exist, however there is a generally persistent inclusion acknowledgement the importance holistic elements such as environment relationships. Integration physical, social, aspects an individual, through social determinants important component in establishing effective delivery optimal care. With increasing numbers collaborative care teams, promotion strategies, primary increasingly building its capacity to help respond these needs, with increased more purposeful attention health. Despite steps right direction, gap continues exist many people continue struggle accessing adequate treatment. In order determine how best proceed, it understand what is, looks like, existing challenges are, other models have been successful integrating into system.
