Cervical cancer screening by simple visual inspection after acetic acid.

摘要: OBJECTIVE: To estimate the sensitivity and specificity of visual inspection using acetic acid as a primary screen for cervical intraepithelial neoplasia (CIN). METHODS: Visual was done on 1997 women aged 35–45 years in screening trial rural China. Each had colposcopy at least five biopsies (directed biopsy lesions, one 2, 4, 8, or 10 o’clock squamocolumnar junction each normal quadrant, an endocervical curettage). RESULTS: Forty-three biopsy-proven CIN II, 31 III, 12 invasive cancer. In two only endocervix positive (one with II III). Visual yielded results 1445 (72%), low-grade 525 (26%), high-grade 21 (1%), cancer six (0.3%). With abnormal defined worse, detecting proven worse 71% (61 86, 95% confidence interval [CI] 60%, 80%); 74% (1420 1911, CI 72%, 76%); 65% smaller lesions (37 57, 51%, 77%), 89% larger (24 27, 71%, 98%) (P = .03). CONCLUSION: The equaled exceeded reported rates conventional cytology. have similar profiles greater. The benefit inexpensive point-of-care diagnosis treatment algorithm will be powerful incentive to pursue developing countries.
