摘要: Objectives:The purpose of this study was to survey new enrollees in a community pharmacy, employer-based diabetes and hypertension coaching program describe the characteristics, health beliefs, cues action newly enrolled participants.Methods:A 70-question, 5-point Likert-type developed using constructs from Health Belief Model (HBM), Theory Planned Behavior (TPB), Reasoned Action (TRA). New programs completed survey. Survey responses between controlled uncontrolled patients patient demographics were compared.Results:Between November 2011 2012, 154 Patients fairly well with mean hemoglobin A1C 7.3% blood pressure 134/82 mm Hg.
