Detecting adverse events in Thai hospitals using medical record reviews: agreement among reviewers.

摘要: BACKGROUND Attention to the problem of adverse events increases worldwide. The present study aimed evaluate inter-rater reliability medical record reviews in Thai context. MATERIAL AND METHOD A total 279 inpatient records were reviewed by 23 clinical auditors. Each was examined independently 3 auditors using a standardised review form. Agreements on occurrence AEs among assessed Kappa statistic (kappa). RESULTS Agreement detecting at least 1 condition potentially related moderate (kappa = 0.54, 95% confidence interval (CI) 0.42-0.65). each item listed conditions ranged from slight substantial 0.08 0.79). There positive trend correlation between rate and kappa (Spearman rank 0.65, p-value 0.058). determining fair (weighted 0.34, CI 0.22-0.45). preventability 0.27, 0.16-0.39). CONCLUSION detect is influenced prevalence AEs, as well variability reviewers. use chart AE reasonable if event sufficiently high.
