Orofacial Pain: From a Dental Point of View

摘要: 口腔顔面領域にみられる侵害受容性疼痛は, 深部痛が主体をなし, 容易に関連痛を生じる. この関連痛が鑑別診断を難しいものにしており, しばしば不適当な対応を導いている. 口腔顔面領域の関連痛で最も頻度の高いものは, 咀嚼筋や頸部筋群から生じるもので, 注意深い触診によってトリガーポイントと主症状との関係を証明できれば確定診断が可能となる. 歯科における神経因性疼痛は, 多くが医原性の疼痛で情動の関与もあり, 難治性である. 舌痛症, バーニングマウス症候群は, 精神疾患との関連も考えられるが, 末梢神経傷害の可能性も指摘されている. 口腔顔面痛の多くは, いまだ機序が十分に解明されていない. 今後さらなる解明が望まれる.
