Comparison of intravenous nitroglycerin and sodium nitroprusside for treatment of acute hypertension developing after coronary artery bypass surgery.

摘要: The present study was designed to test the hypothesis that i.v. nitroglycerin is as effective sodium nitroprusside for managing acute hypertension early after coronary artery bypass surgery. Seventeen patients received both and in a randomized crossover protocol. Infusion rates were increased stepwise lower mean arterial pressures comparably with each drug. In 14 of 17 patients, similar infusion two vasodilators resulted equal lowering blood pressure systemic vascular resistance. remaining three very high required achieved only 20-50% nitroprusside's response three. Hemodynamic responses similar, except cardiac output more than did. contrast, pulmonary gas exchange differed improved intrapulmonary shunting, while worsened it. Similarly, significantly smaller increase alveolar oxygen gradient did nitroprusside. These results suggest majority controlling addition, appeared have favorable effects on exchange. Because has beneficial intercoronary collateral flow setting regional ischemia, it may be preferable ischemic heart disease.
