摘要: The Seamless Transitions and (Re)admissions Network (STARNet) met in December 2012 to synthesize ongoing hospital-to-home transition work, discuss goals, develop a plan centralize information the future. STARNet participants consisted of experts field pediatric hospital medicine quality improvement research, included physicians key stakeholders from groups, private payers, as well representatives current collaboratives. In this report, we (1) review knowledge regarding transitions; (2) outline challenges measuring reducing readmissions; (3) highlight research gaps list potential measures for quality. with support American Academy Pediatrics’ Quality Improvement Innovation Networks Section on Hospital Medicine.
