Exploring perinatal shift-to-shift handover communication and process: an observational study.

摘要: Rationale, aims and objectives: Loss of situation awareness (SA) by health professionals during handover is a major threat to patient safety in perinatal care. SA refers knowing what going on around. Adequate communication process may support assessment, precursor SA. This study describes current practices opinions identify potential improvements. Methods: Structured direct observations shift-to-shift handovers (n = 70) an academic setting were used measure (presence order levels SA: situation, background, assessment recommendation) (duration, interruptions/distractions, eye contact, active inquiry reading information back). Afterwards, receivers' (n = 51) measured means questionnaire. Results: All present 7% handovers, the 86%, background 99%, 24% recommendation 46%. In 77% was mentioned first, followed situation. Forty-four per cent took 2 minutes or more per patient. 52% distractions occurred, 43% there no inquiry, 32% contact 97% not read back. The overall mean 4.1 (standard deviation ± 0.7; scale 1–5, where 5 excellent). Conclusions: Perinatal are currently at risk for inadequate because variability limitations process. However, very positive, indicating lack threats handover. Therefore, staff's should be raised, example through video reflection simulation training. (aut.ref.)
