摘要: Many patients consider the interface between secondary and primary care difficult, in particular, transition of these different parts healthcare system presents problems. This has long been recognized as a critical point for quality care. The purpose our study is to formulate solutions problems identified by cancer professionals during from hospital back general practice on completion treatment cancer.
