摘要: . Objectives: To determine the prevalence of occult valvular pathology in afebrile injection drug users (IDUs) compared with an afebrile, non-IDU population. characterize type present light current recommendations regarding periprocedural antibiotic prophylaxis against endocarditis. Methods: This was a comparative, cross-sectional study involving convenience sample 98 patients history use, and 99 presenting to large urban ED. Patients were excluded if they had one or more following: cardiac valve abnormality, endocarditis, fever on presentation, murmur. Data collected that included demographics, medical history, details use. Transthoracic echocardiography (echo) performed each subject ED, read blinded fashion by single board-certified cardiologist. Results: Among IDUs non-IDUs, 12% aortic thickening, 5.1% non-IDUs (99% CI for difference 7.8%= -3.0% 18.6%). Forty-four percent mitral 25% 18.3%= 0.9% 35.7%). Eleven vs 1% tricuspid thickening 10.4%= 1.6% 19.2%). No patient pulmonic thickening. Six 0% annulus 6.3%= -0.1% 12.8%). Twelve 3% chordae 9.5%= -0.4% 19.4%). Tricuspid recorded 2% 2.1%= -1.7% 6.0%). Most important, regurgitation small, evenly distributed two groups. vegetations seen. Conclusion: Both have pathology. There is increased The regurgitation, reported indication prophylaxis, small affected valves not statistically different between These findings question selected routine use IDU undergoing invasive procedures.
