Effect of germinal matrix hemorrhage on terminal vein position and patency.

摘要: PURPOSE: Pathologic studies have suggested that periventricular intraparenchymal hemorrhages (IPH) occur as the result of compression and occlusion terminal veins (TV) by germinal matrix (GMH). This study used color Doppler ultrasound techniques to test this hypothesis in vivo. MATERIALS AND METHODS: Twenty-four infants with 32 intracranial (20 GMH, 12 GMH/IPH) were evaluated during routine sonography duplex using an ACUSON 128 XP unit 7-MHz linear vector transducers. Maximum transverse dimension was obtained for each GMH. On coronal images, TVs displacement, presence flow, time average velocity (TAV). RESULTS: Abnormalities TV common ipsilateral Of 48 evaluated, no flow identified 13 (27 %), a displaced vein seen eight (17 %). abnormality occurred more frequently GMH/IPH (90 %) than GMH alone (52 complete (82 (16 %, p < 0.002). size correlated increasing abnormality. Mean dimensions unaffected TV, occluded 8.2 +/- 1 mm, 10.3 12.6 respectively (p = 0.008). A small but significant trend observed between maximum decreasing velocities (r - 0.5, 0.01). CONCLUSIONS: Obstruction hemorrhage may play important role pathogenesis white matter hemorrhage.
