Predictors of vertigo in the emergency department: The preved study.

摘要: Abstract Background and Purpose Acute vertigo (sense of motion) can be the sole manifestation a posterior circulation stroke, often gets missed in emergency department (ED). The studies for evaluation central have focused on physical exam findings, which require expertise may not suitable rapid triage by nurse ED or paramedics. Methods This cross sectional study included retrospective chart review patients 18 years age older who presented to Adult with acute dizziness during calendar year 2017. All diagnosis peripheral were final analysis. Sensitivity, specificity, Likelihood Ratio positive result (LR (+)) negative (-)) calculated risk factors, symptoms examination features. Chi-squared test univariate logistic regression used evaluate statistical correlation calculate prevalence odds ratio (POR). Results Two hundred forty nine out 505 (49.3%) presenting had vertigo. Of these, 14 163 Statistically significant variables were: constant (p 0.000- POR 8.7, 95% confidence interval (CI) 2.3-33.1), no change head movement 10.2, CI 3.0-35.4), dysmetria 56.8, 5.8-557.1), unsteady gait 13.3, 3.3-54.3). sensitivity specificity detect 100% 66.4% respectively if patient either gait, symptoms, movement, [VAIN triad (Vertigo- Ataxia, Incessant, Non-positional)]. Conclusions We suggest that VAIN design prospective develop algorithm detection ED.
