Quality Indicators Using Hospital Discharge Data: State and National Applications

摘要: Article-at-a-Glance Background Demand for information about the quality of health care has escalated. Yet many organizations lack well-specified measures, statistical expertise, or requisite data to produce such information. The Healthcare Cost and Utilization Project Quality Indicators (HCUP QIs) represent one approach measuring using readily available on hospital inpatients. Methods HCUP QIs, developed in 1994, address clinical performance rather than other dimensions as satisfaction efficiency. 33 indicators rates that measures outcomes (mortality complications), utilization, access. In lieu complex multivariate techniques, two methods were used: (1) restrictions defining patient subgroups isolate homogeneous at-risk populations (2) standardization when are diverse. Stratified analyses recommended factors believed influence outcome. A simple method making comparisons national was developed. Ql software, both mainframe microcomputer applications, have enabled use their own comparative statistics examine trends over time. Results summarized at individual aggregate level being used stimulate continuous improvement initiatives. Conclusions QIs offer a low-cost alternative access administrative data. Current users include associations, state departments, statewide organizations, hospitals. Although intended serve indicators, not definitive quality, they designed highlight concerns target areas more intensive study.
