摘要: Abstract Glomerular function was monitored prospectively in 13 patients with insulin-dependent diabetes and diabetic nephropathy for up to 51 months. filtration rate, measured by Cr-EDTA clearance, showed a linear decline all patients. Rates of fall ranged between 0.63 2.4 ml/minute per month (mean ± SEM 1.2 0.16). Plasma creatinine concentration proved be an insensitive marker glomerular function, especially the early phase nephropathy. A good correlation found rate change that inverse levels when plasma concentrations exceeded 200 μmol/liter. Inverse beta 2 -microglobulin concentrations, however, highly significant (r = 0.93; p over whole range values, making it sensitive screening impairment renal function. relationship 0.85; -mlcrogiobulin above 3 mg/liter. progressive increase fractional clearance albumin, IgG, noted as fell, indicating evolving defect handling proteins. The unrelated age, sex, duration diabetes, before onset proteinuria, initial albumin blood glucose control, arterial pressure, diastolic values were below 100 mm Hg. effect therapeutic intervention (e.g., glucose, or diet) on progression can reliably evaluated precise measures changes factor(s) determining individual still remain obscure.
