脊柱管内の出来事—Neuraxial anesthesiaに影響する現象—

摘要: Neuraxial anesthesia［脊髄くも膜下麻酔 (脊麻) や硬膜外麻酔 (硬麻) ］に関連する脊柱管内の形態学的な問題については, これまであまり注目されることがなかった. 筆者らはミエログラフィを用いて研究を行い, 脊髄くも膜下硬膜外併用麻酔 (脊硬麻) 時の硬膜外腔容量効果を明らかにした. さらに, 硬膜外注入や妊婦で起こる脊柱管内の変化を脊髄MRIを用いて観察した. 次に, 筆者らは体位変換に伴って脊髄馬尾神経がくも膜下腔内で移動することを, MRIおよび献体を用いた脊髄の解剖を行い確認した. この馬尾神経の動きは成人と小児で違いがみられた. 本稿では, これらの脊柱管内の現象について概説する.
