Traumatic dissection of the internal maxillary artery associated with isolated glossopharyngeal nerve palsy: case report.

摘要: OBJECTIVE AND IMPORTANCE Spontaneous or traumatic dissection of the internal carotid artery with resultant lower cranial nerve palsies is well documented. However, external has not been reported previously. CLINICAL PRESENTATION A 42-year-old man experienced an epidural hematoma as result a fall and underwent craniotomy removal. Subsequently, he developed dysgeusia difficulty in swallowing. Brain magnetic resonance imaging showed dilated linear structure, isosignal intensity on T1-weighted images hyperintense signal T2-weighted images. Strong enhancement was seen postcontrast images, indicating dissected maxillary artery. This confirmed selective angiography left common INTERVENTION Guglielmi detachable coils were introduced into false lumen 0.5 ml glue mixed Lipiodol (Lafayette Pharmacal, Lafayette, IN) packed remnant lumen. Repeat angiograms demonstrated complete occlusion vessel. The patient's postoperative course uneventful, neurological deficits gradually improved. CONCLUSION We describe first case pseudoaneurysm presenting isolated glossopharyngeal palsy. association between palsy branch may be compressive mechanism, suggested by its anatomic relationships, characteristics dissection, good prognosis. Endovascular embolization effective therapeutic method for improving alleviating produced mass effect.
