There were retrospective design hospitalized patient data 2 years which all population taken sample; cross-sectional outpatient care using purposive sampling methods collect sample 3 months. Drug recorded evaluated DIPS (Drug Interaction Probability Scale) determine causation interaction. Patients involved this 60 22 outpatients. total numbers occured 6 actual cases 39 potential cases. Those include 60% 13.6% outpatient. outcomes from observation showed that interaction occurred mostly caused by medication although it’s not harmful. Pharmacist’s role needed monitoring minimize prevent interactions. Polypharmacy represent health hazards patient.
