摘要: Purpose To investigate the effectiveness of a centralized, nurse-delivered telephone-based service to improve care coordination and patient-reported outcomes after surgery for colorectal cancer. Patients Methods with newly diagnosed cancer were randomly assigned CONNECT intervention or usual care. Intervention-group patients received standardized calls from centrally based nurse 3 10 days 1, 3, 6 months discharge hospital. Unmet supportive needs, experience coordination, unplanned readmissions, emergency department presentations, distress, quality life (QOL) assessed by questionnaire at months.
