In Vitro Efficacy of Flomoxef against Extended-Spectrum Beta-Lactamase-Producing Escherichia coli and Klebsiella pneumoniae Associated with Urinary Tract Infections in Malaysia.

摘要: The increasing prevalence of extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae has greatly affected the clinical efficacy β-lactam antibiotics in management urinary tract infections (UTIs). limited treatment options have resulted increased use carbapenem. However, flomoxef could be a potential carbapenem-sparing strategy for UTIs caused by ESBL-producers. Here, we compared vitro susceptibility UTI-associated ESBL-producers to and established antibiotics. Fifty Escherichia coli Klebsiella pneumoniae strains isolated from urine samples were subjected broth microdilution assay, presence ESBL genes was detected polymerase chain reactions. High rates resistance amoxicillin-clavulanate (76–80%), ticarcillin-clavulanate (58–76%), piperacillin-tazobactam (48–50%) observed, indicated high minimum inhibitory concentration (MIC) values (32 µg/mL 128 µg/mL) both species. blaCTX-M blaTEM E. (58% 54%, respectively) K. (88% 74%, respectively), whereas blaSHV found only (94%). Carbapenems remained as most effective against ESBL-producing associated with UTIs, followed cephamycins. In conclusion, may alternative carbapenem Malaysia.
