Intracardiac Electrophysiologic Techniques in Recurrent Syncope of Unknown Cause

摘要: Twenty-five patients with recurrent episodes of syncope, unexplained despite thorough medical and neurologic evaluation, underwent intracardiac electrophysiologic study programmed stimulation. Electrophysiologic yielded a presumptive diagnosis in 17 patients: nine rapid ventricular tachycardia by stimulation, three intra-His conduction delays, one symptomatic atrial flutter, sick sinus syndrome, persistent hypervagotonia manifested as atropine-reversible prolongation atrioventricular nodal refractoriness. Therapy based on these findings provided complete relief 14 improvement during mean follow-up 18 +/- 10 months. testing was ineffective two the patients. Syncope persisted four eight whom did not define probable arrhythmic mechanism. These observations indicate that full evaluation stimulation is useful therapy syncope.
