Résultats de la duodénopancréatectomie chez les octogénaires : expérience monocentrique et revue de la littérature

摘要: Resume Introduction Les cancers du pancreas et de la region peri-ampullaire chez les patients âges plus 80 ans posent encore des problemes aux chirurgiens a cause risque chirurgie sur ce terrain. Nous avons revu notre experience celle rapportee dans litterature pancreatectomies octogenaires, afin d’evaluer resultats precoces survie long terme en cas cancer. Methode Trois cent quatre-vingt-cinq ayant eu une pancreatectomie pour tumeur entre 1998 et 2011 etaient inclus l’etude ont ete divises deux groupes : le groupe 1 forme moins 80 ans ; 2 de plus. La morbidite, mortalite perioperatoires, sans recidive etaient analysees. Une recherche bibliographique etait faite resections pancreatiques octogenaires. Resultats Parmi 385 patients : 362 etaient 1 et 23 dans 2. Il n’y avait pas differences significatives sexe caracteristiques tumorales groupes. Le taux complications (40 vs 43 %) (4 vs 0 %) globale mediane cancer (21 vs 19 mois) n’etaient significativement differentes revue retrouve 14 etudes dont plupart etudes (surtout celles provenant centre gros volume) montraient similaires octogenaires jeunes. Conclusion est option acceptable âges. L’âge seul ne peut etre considere comme contre-indication resection pancreatique, mais evaluation rigoureuse avant l’intervention prise charge perioperatoire optimale sont essentielles.
