Use of D-dimer and lower extremity Doppler ultrasound results to obviate the need for computerised tomographic pulmonary angiography.

摘要: Introduction: We hypothesise that correct interpretation of other diagnostic tests could reduce the use computerised tomographic pulmonary angiogram (CTPA) examinations in patients with suspected embolism (PE). Materials and Methods: carried out a retrospective analysis 158 928-bed university hospital. These consecutive were investigated for PE from May 2001 to February 2002 using CTPA. Results: There 74 men 84 women mean (±SD) age 57 (±19) years. Overall, 56% (89/158) showed clinically significant abnormalities on CTPA examination. The overall prevalence was 15% (24/158). D-dimers assayed 40% (63/158) lower limbs scanned Doppler ultrasound (US) 22% (35/158) patients. None 19 negative D-dimer assays had PE. Of who positively tested US, 4 positive 1 US test. In retrospect, patient management based extremity studies have reduced need further investigation by Conclusion: PE, leg may demand CTPAs. Ann Acad Med Singapore 2006;35:858-63
