摘要: Article-at-a-Glance Background Care transitions across health care settings are common and can result in adverse outcomes for older adults. Few studies have examined professionals' perspectives on important process measures or pay-for-performance (P4P) strategies related to transitional care. A study was conducted characterize (1) successful of adults (age 65 years older), (2) suggestions improvement, (3) P4P Methods In a qualitative study, one-hour semistructured in-depth interviews were an acute hospital, skilled nursing facility, two community-based primary practices, one home agency with 20 professionals (18 physicians 2 administrators) direct experience who likely be affected by strategies.
