Patient, provider and system factors influencing rehospitalisation in adults with heart failure

摘要: Objectives: To identify patient, provider and system factors predicting rehospitalisation in adults with heart failure (HF). Method: The electronic data bases MEDLINE, Cumulative Index to Nursing Allied Health Literature (CINAHL), Embase PsychInfo were searched from 1996–2008 studies identifying predictors of which the systems extracted. Results: A total 62 elements identified. These then sorted into categories. Risk identified for predominantly related clinical factors. There less risk perspective health care systems. Conclusions: In people HF it is likely that non-clinical including play an important role rehospitalisation. a need individuals at tailor disease management programmes accordingly.
