摘要: OBJECTIVE: To evaluate the frequency of ocular complications and clinical outcomes these in patients with various stages HIV infection. METHODS: Retrospective review all HIV-infected seen an AIDS ophthalmology clinic from November 1983 through December 31, 1992. RESULTS: Eleven-hundred sixty-three were for ophthalmologic evaluation. Of these, 781 had acquired immune deficiency syndrome (AIDS), 226 symptomatic infection (AIDs-related complex [ARC]), 156 asymptomatic Non-infectious retinopathy was most common complication, affecting 50% AIDS, 34% ARC, 3% Cytomegalovirus (CMV) retinitis opportunistic infection, 37% AIDS. Other infections, including toxoplasmosis, varicella zoster virus retinitis, Pneumocystis choroidopathy much less common, each occurring < or = 1% Treatment CMV either foscarnet ganciclovir successful initially controlling retinitis. However, relapse represented a significant problem required frequent re-inductions. As consequence retinal damage associated relapse, loss visual acuity occurred. The median time to 20/200 worse eyes 13.4 months, better eye 21.1 months. At last follow-up, 75% final 20/40 at least one eye. Retinal detachments complication cumulative probability detachment 57% 12 months after diagnosis Herpes ophthalmicus developed overall series Fifty-six percent cases toxoplasmosis simultaneous toxoplasmic cerebritis. Ocular responded standard anti-microbial therapy. Varicella when manifested by acute necrosis (ARN) syndrome, intravenous acyclovir Conversely, limited number progressive outer disease poorly therapy, but appeared respond combination Neuro-ophthalmic lesions present 6% cause neuro-ophthalmic lesion cryptococcal meningitis, 25% meningitis complication. CONCLUSIONS: manifestations are major vision-threatening patients. While available therapy phenomenon resulted some degree long-term loss. Preservation patient's generally successful. infections substantially than require aggressive
