Underestimating the Effect of Lipids on Cardiovascular Events: Regression Dilution Bias in the Population-Based Cohort of Tehran Lipid and Glucose Study.

摘要: Background: Random errors in the measurement of risk factors lead to bias exposure-disease association. Objectives: This study aimed examine extent underestimation association total cholesterol (TC), high density lipoprotein (HDL-C) and triglyceride (TG) with cardiovascular disease (CVDs) Tehran Lipid Glucose Study. Patients Methods: Of 6327 eligible people original cohort followed for about 10 years detect CVD events, 3063 (1224 men 1839 women) had replicate measurements blood lipids. Two regression dilution ratios were calculated by nonparametric parametric methods, using data from reexaminations three six after baseline. Adjusted unadjusted hazard (HR) corrected bias. Results: By method, based on reexamination baseline, strength real usual levels TC, TG HDL-C disease, considering β coefficients related models, underestimated 42%, 51% 81% 40%, 43% 62% women, respectively. Underestimations relatively independent age, sex body mass index. Correction led more than 60% increase estimated HR TC adjusted confounders. Conclusions: Using baseline lipids considerable these outcome TLGS. The increased time interval between follow-up TG. attenuation due stronger relation risk.
