摘要: Use of patient safety reporting systems (PSRS) to identify and mitigate risks patients who are harmed by medical care has been a national priority for nearly decade. Yet, most still new focus on events. To improve the value PSRS, we must use data hazards, prioritize where resources, develop interventions these evaluate whether reduced harm. We developed implemented Web-based PSRS discuss in this paper benefits, limitations, challenges encountered. First, benefits as part learning community. The remainder focuses faced that need be resolved systems. address follows: what report, how minimize burden costs, conduct expert reviews efforts, place incidents into taxonomies, know system actually improved safety, should responsible attempting risk mitigation.
