Combined laparoscopic and endoscopic approach in patients with cholelithiasis and choledocholithiasis

摘要: Background. The optimal management of common bile duct stones in patients undergoing laparoscopic cholecystectomy remains controversial. Methods. A prospective study was conducted 145 the 481 who had a preoperative endoscopic retrograde cholangiogram before their cholecystectomy. Results. Endoscopic successful 138 (95%), and calculi were found 72 (50%) them. sphincterotomy with ductal clearance achieved 62 67 during mean 1.4 sessions (range, 1 to 5). Five (3.4%) complications after intervention, all which resolved uneventfully. Fourteen underwent exploration, five failed extraction, left intentionally for addition, four seven identified by intraoperative cholangiogram. Ten these 14 exploration. Laparoscopic successfully completed 134 patients, none major or postoperative complications. stay 2.7 days those procedure. overall number admissions completing treatment 2.3. Conclusions. Combined approach is viable option gallstones choledocholithiasis.
