摘要: Concentrations of vancomycin in bones 14 patients undergoing total hip arthroplasty (group 1) and 5 with osteomyelitis 2) were studied. Group 1 received vancomycin, 15 mg/kg intravenously, h prior to anesthesia. 2 doses adjusted achieve peak levels serum 20 30 micrograms/ml trough less than 12 micrograms/ml; bone specimens collected during surgical debridement. The pulverized eluted into phosphate buffer, the supernatants analyzed for content by fluorescence polarization immunoassay. In group 1, was detectable all cancellous a mean concentration 2.3 +/- 4.0 micrograms/g (range, 0.5 16 micrograms/g); 10 cortical had vancomycin; 1.1 0.8 not 2.6 micrograms/g). 2, only two five (mean concentration, 5.9 3.5 Cancellous obtained one patient; 3.6 micrograms/g. most higher MIC susceptible staphylococci following single prophylactic doses. few infected studied, penetration variable deserves further study.
