Measurement of Integrated Care on the level of Regional Health System in Singapore

摘要: In response to the rapidly aging population of Singapore and increasing demands on healthcare, healthcare system was reorganized into six regional health systems (RHS) according geographic locations provide a better integrated system. each RHS, an acute hospital works closely with community hospital, nursing homes, homecare providers, general practitioners polyclinics deliver seamless holistic care patients. However, even awareness needs integrate system, there is no universal definition method care. Furthermore, recent initiative RHS has not been evaluated.
