摘要: Background The objective of this study was to analyze the results open heart mitral valve operations for survival prediction in patients with previously unsuccessful MitraClip procedures. Methods Thirty-three consecutive who underwent surgery our institution were studied. At a median 41 days, they had undergone one five futile implantations. time their operations, 72.6 ± 10.3 years old, and calculated risk, using European System Cardiac Operative Risk Evaluation (EuroSCORE) II, 26.5%. Individual outcomes recorded, all monitored postoperatively. Results Thirty-day mortality 9.1%, overall at 2.2 60.6%. Seven cardiac-related six noncardiac deaths occurred. Univariate regression models demonstrated significant influence following variables on survival: EuroSCORE II ( p = 0.0022), preoperative left ventricular end-diastolic dimension ( = 0.0052), ejection fraction 0.0249), coronary artery disease 0.0385), severe pulmonary hypertension 0.0431). Survivors showed considerable improvements New York Heart Association class 0.0080), grade regurgitation 0.0350), area 0.0486). Survival after repair not superior replacement. Conclusions Indications failed procedures must be considered greatest care. Variables predicting postoperative should taken into account regarding difficult decision as whether operate or not. Our data suggest that replacement pretreated is probably more reasonable concept rather than complex repairs. When II exceeds 30%, conservative therapy advisable.
