Unfortunately, the repertoire of agents available to clinicians tasked with treating relapsed or refractory extensive-stage SCLC (ES-SCLC) limited. We report case a patient extensive, brain metastases who had exceptional initial response temozolomide (TMZ). describe also ongoing, durable complete radiographic in excess four years achieved via combination long-term therapy TMZ and selectively deployed stereotactic radiosurgery (SRS). In spite its recent addition National Comprehensive Cancer Network (NCCN) guidelines for SCLC, remains underutilized this indication. This supports off-label use especially those central nervous system (CNS) metastases--for whom there are few no treatment options.
