A prospective controlled study of the effect of intramural uterine fibroids on the outcome of assisted conception.

摘要: BACKGROUND: Although uterine fibroids occur in 30% of women and are associated with a degree subfertility, the effect intramural on outcome IVF or ICSI treatment has not been prospectively studied. METHODS: Data were collected 434 undergoing IVF/ICSI assisted conception unit an inner London teaching hospital. Patients assessed for presence by transvaginal ultrasound hysterosonography hysteroscopy where appropriate. RESULTS: During study period, 112 (study), 322 without (controls), treated. similar regarding cause duration their infertility, number previous treatments, basal serum FSH concentration. Women group average 2 years older (36.4 versus 34.6 years; P < 0.01). There was no significant difference ovarian stimulation gonadotrophin requirement, follicles developed, oocytes collected, embryos available transfer replaced. When analysing only ≤5 cm size (n = 106) pregnancy, implantation ongoing pregnancy rates significantly reduced: 23.3, 11.9 15.1 respectively compared 34.1, 20.2 28.3% control (P 0.016, 0.018 0.003). The mean largest 2.3 (90% range 2.1-2.5 cm). Logistic regression analysis demonstrated that one variables affecting chance even after controlling replacement increasing age, particularly age ≥40 years, odds ratio 0.46 (CI 0.24-0.88; 0.019). CONCLUSION: This fibroid halves chances following conception.
