摘要: BACKGROUND: The percentage of patients with office or white-coat hypertension has been reported in international studies to be 20-30% the hypertensive population. These can identified and distinguished from established by ambulatory blood pressure monitoring (ABPM) self-measurement. There is still no satisfactory explanation for phenomenon 'office hypertension' there are data available show how behaviour hypertensives differs that normotensive subjects away physician's clinic environment. OBJECTIVE: To investigate clinical setting over a 24 h period compare it subjects. DESIGN AND METHODS: heart rate 36 hyhpertension 33 matched age sex were measured outpatient using ABPM. subjected both conventional modern rhythm analysis. Urinary catecholamine excretion was as marker sympathetic activity. RESULTS: In clinic, had significantly (P < 0.01) higher values (146/97 mmHg) than did controls (128/81 mmHg). analysis well ABPM revealed difference between two groups respect h, daytime night-time mean SD. However, showed maximum lower minimum value systolic diastolic pressures hypertension, resulting greater amplitude due 0.005 P 0.05) minus (38/32 compared those (29/28 early morning rise 0.008 0.03) (11/9 versus 7/7 intersected curve at approximately 0600 h. No significant differences any time observed groups. urinary noradrenaline dopamine 0.01 increased during hypertensives. CONCLUSION: Patients who definition do not yet have already exhibit abnormal regulation daytime, (amplitude hypertension) day activation. Office seems earliest manifestation hypertension.
