摘要: Introduction: Recent clinical data support the utility of left heart pacing. The transvenous approach for pacing lead implantation is imperfect. A direct epicardial may have advantages, but heretofore its has been limited because requirement thoracotomy. We sought to examine feasibility a method that did not require thoracotomy. Methods and Results: In five large swine, percutaneous access epicardium was achieved with subxiphoid videopericardioscopy, using device marries endoscopy port through which leads could be introduced. each animal, standard, active fixation were implanted onto atrium ventricle. atrial at base appendage. ventricular on anterior, lateral, inferior walls. Continuous visualization provided guidance localization fixation, including avoidance complications such as trauma coronary vessels. Capture thresholds uniformly low.
