Early Gallbladder Cancer: Clinical, Morphological, Therapeutic and Evolutionary Aspects

摘要: Introduction: The early cancer of gallbladder is an entity which not well recognized currently. It a does extend beyond the muscularis layer the and it characterized in almost cases by absence lymph node visceral invasion. Patients Method: We have conducted this retrospective study of all our cases treated surgical unit. have studied these through clinical, morphological, therapeutical evolutionary aspects. Results: Of 202 gallbladder carcinoma, 33 cancers were classified as cancer. 25 females 8 were males. mean age was 56.4 years (41 - 70 years). All patients free symptoms all except one had normal CEA CA19.9. 2 had synchronous tumors (one colonic cancer and rectal cancer). For 16 patients, diagnosis was done by ultrasonography 17 histological examination specimen removed for biliary lithiasis. 8 PT1a tumor (confined only to mucosa) PT1b (tumor infiltration muscular layer). For 19 who benefited from extensive lymphadenectomy, only one (5.3%) infiltration. simple cholecystectomy two cases, cholecystectomy associated with bile duct resection. hepatectomy lymphadenectomy. patients had simultaneous right colectomy abdominoperineal resection another one choledocal cyst 3 stone removal bile duct removal (ruptured tumor in duct). 1 patient (3.7%) died postoperative course (hospital mortality). In follow-up period, 4 intercurrent causes. Two patients presented recurrence at 14 36 months respectively 19 42 months. One presented 66 months. She at 78 after palliative treatment. Currently, 22 patients (66.7%) are still alive without median survival 53 and 31 Conclusion: Early gallbladder must be known radiologist and surgeon. Recognized on time well treated, cancer can cured its prognosis excellent.
