Brodifacoum pharmacokinetics in acute human poisoning: implications for estimating duration of vitamin K therapy

摘要: Standard of care follow-up therapy for patients poisoned by long-acting anticoagulant rodenticides (LAARs) is daily high-dose (up to 100 mg per day) oral vitamin K1 (VK1) weeks months over a year. The availability CLIA-certified quantitative testing plasma LAAR concentrations can now assist health providers in determining when safely discontinue VK1 therapy. We present estimates treatment duration required reach safe (< =10ng/ml) using serial measurements brodifacoum (BDF, potent LAAR) obtained from after inhaling synthetic cannabinoids containing BDF. fit the data zero-order (linear) and first-order (exponential) curves, latter account enterohepatic circulation results show that are significantly longer exponential clearance assumed. Accordingly, we recommend BDF be monitored simultaneously with international normalization ratio (INR) during patients, determined discontinued document persistence concentrations.
