The effect of spinal anesthesia on blood transfusion rate in total joint arthroplasty

摘要: Contexte : Les patients qui subissent une arthroplastie totale (AT) recoivent souvent transfusion de sang allogene. On croit que l'anesthesie regionale (AR) protege contre le besoin sanguine, mais beaucoup d'etudes randomisees portant sur l'AR dans les cas d'AT n'ont pas atteint cette conclusion a l'unanimite. Nous voulions decrire l'effet d'une AR la allogene contexte importante serie retrospective d'AT. Methodes avons analyse des donnees provenant toutes AT pratiquees Edmonton (Alberta) en 2000 (n = 1875) et nous utilise modelisation par regression logistique pour determiner lien entre l'utilisation Resultats Vingt-huit cent ont subi recu etabli un independant l'âge croissant, baisse masse corporelle, diminution du taux d'hemoglobine preoperatoire, sexe feminin, comorbidite accrue duree prolongee l'operation. Compte tenu ces facteurs, constate recours (sous forme d'anesthesic rachidienne) comparativement generale reduit coefficient probabilite (CP) 0,729 (intervalle confiance 95 % [IC], 0,559-0,949). Le resultat represente combinaison d'un solide prevention d'arthroplastie hanche (CP 0,646, IC %, 0,443-0,944) non significatif genou 0,825, 0,564-1,208). Conclusion L'anesthesie rachidienne genou. est conforme avec ce l'on sait consequences hemodynamiques rachidienne.
