摘要: Objective Analyze the economic value of replacing conventional fetal aneuploidy screening approaches with non-invasive prenatal testing (NIPT) in general pregnancy population. Methods Using decision-analysis modeling, we compared to NIPT cell-free DNA (cfDNA) analysis annual US population. Sensitivity and specificity for aneuploidies, trisomy 21, 18, 13, monosomy X, were estimated using published data modeling both first- second trimester screening. Costs assigned each test component an affected birth. The overall cost healthcare system considered costs, number aneuploid cases detected, invasive procedures performed, procedure-related euploid losses, pregnancies averted. analyses evaluated effect variation parameters. reported 2014 Dollars. Results Replacing would reduce costs if it can be provided $744 or less most influential variables timing entry, termination rates. Of 13,176 undergoing screening, detected 96.5% (12,717/13,176) cases, 85.9% (11,314/13,176) by approaches. reduced 60.0%, methods resulting 24,596 61,430 procedures, respectively. losses was 73.5% (194/264) population. Conclusion Based on our analysis, universal application increase detection rates economically justified. Offering this all pregnant women is associated substantial benefits.
